If proof of income is not provided services will be charged at the full 100 rate.
To apply you must meet all minimum requirements and submit a Pitt County or NC State PD107 application to the Human Resources Office. Applications can be completed online or submitted to the Human Resources Office. Always bring proof of family income when receiving services from the Pitt County Health Department.
